Medicare has adopted the following definitions. BOCFS are encouraged to follow Medicare’s requirements when providing footwear and inserts as a protocol for Diabetic foot care.

A.     Non-custom Therapeutic Shoes are Medicare approved off-the-shelf, unmodified depth-inlay shoes manufactured to accommodate multi-density inserts.

B.     Non-custom Diabetic Multi-density Inserts are Medicare approved prefabricated multiple density accommodative foot orthotics, direct formed, molded to foot after external heat source of 230 degrees Fahrenheit or higher, total contact with patient’s foot, including arch, base layer minimum of ¼ inch material of shore a 35 durometer or 3/16 inch material of a shore a 40 (or higher).

C.     Non-therapeutic Shoes & Non-therapeutic devices are defined as prefabricated unmodified and/or un-modifiable foot care and footwear products, non-therapeutic, accommodative inlays and non-therapeutic accommodative footwear, regardless of method of manufacture and unmodified, non-therapeutic over-the-counter shoes. For purposes of this document, “accommodative” means designed with a primary goal of conforming to the individual’s anatomy, and “therapeutic” devices are devices that address a medical condition, while non-therapeutic devices are devices that do not address a medical condition.
